Cagrilintide Overview
Cagrilinitide is a long-acting, lipidated, albumin-binding amylin agonist that activates both amylin and calcitonin receptors. It is designed for obesity management and may be used alone or paired with a GLP-1 receptor agonist.
Amylin, naturally co-secreted with insulin, plays a key role in appetite control, satiety, gastric emptying, and glucose regulation.
Why Combine Cagrilinitide With a GLP-1?
When used together with a GLP-1 such as Semaglutide, Cagrilinitide produces a strong synergistic effect—enhancing weight loss significantly more than either therapy alone. Additional benefits reported include improved circulation, reduced blood pressure, enhanced bone formation activity, and vasodilation.
6–10% total weight loss at 26 weeks (Cagrilinitide alone)
23–24.4% total weight loss at 68 weeks (Cagrilinitide + Semaglutide)
Up to 8% additional weight loss compared to Semaglutide alone
Injection site reactions (redness, swelling, itching, hives)
Nausea or vomiting
Constipation or diarrhea
Fatigue
Headache
Heartburn
Rare: gallbladder stones
Do not use Cagrilinitide if any of the following apply:
Pregnancy or breastfeeding – Safety has not been established.
History of pancreatitis – Amylin agonists may increase risk.
Severe gastrointestinal disorders (e.g., gastroparesis, severe GERD) – Cagri slows gastric emptying.
Uncontrolled thyroid disease – May worsen metabolic instability.
Severe kidney or liver impairment – Clearance and metabolism may be affected.
Active gallbladder disease – Risk of gallstone formation may increase.
Known hypersensitivity to amylin analogs or any component of the formulation.
Active or recent eating disorders (e.g., anorexia, bulimia) – Appetite-suppressing effects may be harmful.
Reconstitution:
Mix Cagrilintide 10mg with 2mL (200 units) of Bacteriostatic Water (BAC Water).
Cycle:
16 weeks ON → 4–6 weeks OFF
Titration Schedule:
Administer subcutaneously, once weekly
Week 1: 8 units (400 mcg)
Weeks 2–3: 15 units (750 mcg)
Weeks 4–5: 24 units (1.2 mg)
Weeks 6–7: 34 units (1.7 mg)
Weeks 8–9: 44 units (2.2 mg)
Weeks 10–16: 54 units (2.7 mg)
Maintain the same dosing day each week
Stay well-hydrated throughout the cycle
Avoid exceeding 16 consecutive weeks
Anti-Cagri antibodies may develop with prolonged use (seen in 46–73% of users)
Semaglutide or Tirzepatide – Maximizes appetite suppression and enhances fat loss
Tesofensine – Increases energy expenditure and thermogenesis
L-Carnitine – Supports fat metabolism and energy conversion
AOD-9604 – Promotes fat breakdown without impacting blood glucose
